Find a Doctor or Pharmacy

If you have a Simply HMO, HMO D-SNP, HMO C-SNP, or HMO I-SNP, you must stay in-network for all of your coordinated health care, except in an emergency, for urgent care situations, or for out-of-area renal dialysis.

If you are enrolled in a Simply HMO, HMO D-SNP, HMO C-SNP, or HMO I-SNP plan and obtain routine care from out-of-network providers, neither Medicare nor Simply Healthcare will be responsible for the costs.

If you have a Simply PPO plan, please refer to your Evidence of Coverage document for your out-of-network options1 and costs. In general, even if you have a Simply PPO plan, your costs will be lower if you stay in-network.

The provider and/or pharmacy network may change at any time. You will receive notice when necessary.

You may request a hard copy of the Provider/Pharmacy Directory to be mailed to you at any time by calling Member Services toll-free number at 877-577-0115 (TTY 711).

Our Member Services are available:

April 1 – September 30, Monday-Friday, 8 a.m. to 8 p.m.
October 1 – March 31, 7 days a week, 8 a.m. to 8 p.m.
We are closed on Thanksgiving and Christmas.

1 Out-of-network/non-contracted providers are under no obligation to treat Plan members, except in emergency situations. Please call our customer service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.
