Am I eligible?

Low-income adults, children and pregnant women who:

• Meet income, resource, age or disability requirements

Children ages 5-18 who:

• Are not eligible for Medicaid
• Are a U.S. citizen or qualified noncitizen
• Meet income requirements

Adults age 18 or older who:

• Are eligible for Medicaid
• Have had a review by Comprehensive Assessment and Review for Long-Term Care Services (CARES) to determine clinical eligibility

To be eligible for SMMC MMA (Medicaid) through Simply, you must live in one of the following counties:

Region 1:
Escambia, Okaloosa, Santa Rosa, and Walton
Region 2:
Bay, Calhoun, Franklin, Gadsden, Gulf, Holmes, Jackson, Jefferson, Leon, Liberty, Madison, Taylor, Wakulla, and Washington
Region 5:
Pasco and Pinellas
Region 6:
Hardee, Highlands, Hillsborough, Manatee, and Polk
Region 7:
Brevard, Orange, Osceola, and Seminole
Region 9:
Indian River, Martin, Okeechobee, Palm Beach,and St. Lucie
Region 10:
Broward
Region 11:
Miami-Dade and Monroe

Simply serves the Florida Healthy Kids program statewide.

To be eligible for Long-Term Care through Simply, you must live in one of the following:
