How to enroll
You have a choice in your health care. Choose Simply for the health care you need plus extra benefits to help you live a healthier life.
Medicaid MMA
1. Apply for Medicaid benefits
To enroll in Simply, you must get Medicaid benefits. If you don’t have Medicaid yet, you can apply online at www.myflorida.com/accessflorida.
2. Choose Simply as your health plan
Once you are approved for Medicaid, you can choose a managed care plan to join. If you don’t choose, you will be assigned to a plan. You can change your plan within the first 120 days of enrollment.
There are two easy ways to enroll with us:
- Online: go to the Florida Statewide Medicaid Managed Care website at www.flmedicaidmanagedcare.com.
- By phone: call the Choice Counseling Helpline toll-free at 1-877-711-3662 (TTY 711) Monday through Thursday from 8 a.m. to 8 p.m. and Friday from 8 a.m. to 7 p.m. An enrollment broker can answer any questions you have.
Bay, Brevard, Broward, Calhoun, Charlotte, Collier, DeSoto, Escambia, Franklin, Gadsden, Glades, Gulf, Hardee,Hendry, Highlands, Hillsborough, Holmes, Indian River, Jackson, Jefferson, Lee, Leon, Liberty, Madison,Manatee, Martin, Miami-Dade, Monroe, Okaloosa, Okeechobee, Orange, Osceola, Palm Beach, Pasco, Pinellas,Polk, Santa Rosa, Sarasota, Seminole, St. Lucie, Taylor, Wakulla, Walton, and Washington counties.

Florida Healthy Kids
A family that does not qualify for Medicaid may still get health care for their child(ren) through Florida Healthy Kids.
1. Apply for benefits
You can apply in one of these ways:
- Online: go to the Florida Healthy Kids website at www.healthykids.org/apply
- By phone: call Florida Healthy Kids at 1-888-540-KIDS (5437) (TTY 1-800-955-8771)
2. Choose Simply as your child’s health plan
Once your child is approved for benefits, you can choose a managed care plan to join. If you don’t choose, your child will be assigned to a plan. Call Florida Healthy Kids at 1-888-540-KIDS (5437) (TTY 1-800-955-8771) for more information on choosing a plan.
Long-Term Care
The Statewide Medicaid Managed Care Long-Term Care (SMMC LTC) program provides services and supports to people who qualify for Medicaid nursing home placement.
Once you are approved for long-term care services, you can choose a managed care plan to join. If you don’t choose, you will be assigned to a plan. You can change your plan within the first 120 days of enrollment.
